1. One passport photo (2" X 2")
One passport photo 2" by 2" is required.
2. Driver’s License or State ID Card
Either a driver’s license or a state ID card is required to process a passport. If you do not have either one, you can use a photo ID such as a college ID or a government employee ID. If you do not have any of these, you must have someone fill out an Affidavit of Identifying Witness (DSP-71) which states the person applying for the passport is truly that person. This form is available at the Clerk of Courts Passport Office. IF YOU SUBMIT AN OUT-OF-STATE DRIVER’S LICENSE, YOU MUST HAVE AN ADDITIONAL FORM OF PHOTO IDENTIFICATION.
If a minor (under 16) is applying for a passport, then he/she must be accompanied by both parents or an appointed legal guardian. The legal guardian must bring the court document stating they are the legal guardian. The parent or legal guardian must have the proper ID with them. If a minor (16 or 17 years old) is applying for a passport the he/she must be accompanied by one parent or an appointed legal guardian.
3. Certificate of Birth (certified copy)
A birth certificate is required to prove U.S. citizenship. If a person does not have a birth certificate, they will need to obtain one by contacting the Vital Statistics Department from the city in which they were born. If that Vital Statistics Department does not have record of the person’s birth, the Vital Statistics Department from the city in which they were born needs to send a letter stating that there is nothing of record in that city. If this is the case, the person will also need an Affidavit from an older living relative stating that the person was born in the city in question.
The letter from the Vital Statistics Department along with the Affidavit from the older living relative must be brought in to verify citizenship. If the two items are brought in, then the person must also bring either a baptismal record, a hospital record, or a school record that has the place of birth listed on it to verify their U.S. citizenship.
NOTE: If a person does not have a birth certificate, they can use a previous passport. (See #4 listed next).
4. Previous Passport
A previous passport may be used for proof of U.S. citizenship instead of a birth certificate. Also, if the passport was issued within the last fifteen (15) years and the person was 16 years or older at the time it was issued, the person can use a mail-in form and apply for a new passport through the mail. These forms are available at our office.
If the passport has expired and it is over fifteen (15) years since it was issued, the person must apply for a new passport in person. The expired passport may still be used for proof of U.S. citizenship instead of a certified copy of a birth certificate.
If the name on the passport has changed, the person must submit a certified copy of the official document which explains why the name was changed. (Example: certified copy of a divorce decree, certified copy of a marriage certificate, etc.) This document is mailed to the New Orleans Passport Agency and this agency returns the document to the individual.
If a minor (under 16) is applying with a previous passport, they need to also submit a birth certificate.
5. Naturalization Document
If a person was not born in the United States, they must submit an original naturalization document to obtain a U.S. passport.
6. Separate Check or Money Order Made Payable to: Clerk of Courts
There is a fee of $35.00 to process a passport. This amount is a separate check or money order made payable to: Clerk of Courts
Only an approved agent can process a passport application. To be an approved agent, you need to be trained and you need to sign a form which is sent to New Orleans Passport Agency.
A passport can be applied for at the Clerk of Courts Office or at the United States Post Office.
A person must come in person to apply for a passport because the signature must be written in the presence of an approved agent.
A standard application form takes 6 to 8 weeks to process.
If you want to expedite an application, it costs an additional $60.00 per person.
An expedited application takes 2 to 3 weeks to process.
If you need a one (1) day processing time period, then you must apply at certain designated states. Example: Chicago, New York, Washington, etc.
