Transportation hours of service are: 5 days per week from 6:00 A.M – 6:00 P.M. with limited service on the weekends. Any other time slots will need pre-approval. Please call transportation provider for information for service hours beyond this.
For service beyond the mileage limits as set for Out-County Medical Transportation (30 miles max), the passenger will pay a rate per mile beyond the limits, as set by the transportation contracts with that transportation provider
To schedule your transportation need, contact the company depending upon the need for wheelchair or non-wheelchair transport. Trips must be scheduled, at the least, 2 days (48 hours) prior to the requested trip. If a scheduled ride needs to be cancelled, please cancel your trip as soon as you are aware of the need to cancel so another person may fill your spot and to assist the transportation provider’s scheduling.
Be prepared to provide the following required information when scheduling a trip:
Name, Date of Birth, and Telephone Number,
Address where you wish to be picked up, if residing in cluster building provide building number or letter,
Full address of your destination including the City and Zip code,
Time you wish to arrive at your destination,
Time you wish to be picked up from your destination,
The return address you wish to be dropped off, and
Any special mobility needs
